PERSONAL INFORMATION PRIVACY POLICY

Prelib inc. and its subsidiaries (“Prélib”) attach great importance to respect the privacy of their patients, their clients, their employees and suppliers. That is why Prelib has implemented this personal information privacy policy (“Policy”) with the aim to govern Prelib’s practices regarding the collection, use and disclosure of personal information.

This Policy refers to how Prelib collects, uses, discloses and protects your personal information. Our practices regarding the respect of your personal information comply with the Loi sur la Protection des renseignements personnels dans le secteur privé du Québec (hereinafter called “PRPSP”). This Policy does not apply to information that does not constitute “personal information” under applicable laws on respect of privacy.


1. COLLECTION AND USE OF PERSONAL INFORMATION

We may collect personal information from you:

  • When you purchase any of our products or services;
  • When you request information about any of our products and services;
  • When you contact Prelib Customer Service or any employees or professionals of Prelib;
  • When you visit the Prelib website or make transaction on said website;
  • When you provide us with comments or suggestions.


2. USE OF PERSONAL INFORMATION

Prelib may use personal information to:

  • Identify customers, employees and suppliers to establish and maintain a relationship with them;
  • Provide customers with ongoing service, to ensure proper services and to better support customers when using our services;

  • Protect against error and fraud;
  • Comply with legal and audit requirements.

Prelib may also use personal information to create statistics about our company. These statistics do not contain information that could personally identify any individual customer, employee or supplier.


3. OTHER USES OF PERSONAL INFORMATION

In the event of all or part of Prélib’s assets were sold or financed, or if Prélib were to merge with another company, personal information in the possession of Prélib could be disclosed to the lender, buyer or company involved in this transaction. However, access and use of such personal information should be in compliance with this Policy and with the laws and regulations applicable to the protection of personal information applicable to professional orders. Prélib undertakes to ensure that any transferee undertakes to comply with this Policy. In addition, all health professionals working in partnership with Prélib are committed to ensuring the protection of personal information under the laws and regulations imposed on them by their respective professional orders.


4. TRANSFERS TO AGENTS AND THIRD PARTIES

Some of the products and services that are provided by Prelib may be provided by third parties in cooperation with Prelib. In order to provide you with the products or services that you request from us, we may have to transfer information about you to these third parties.

Where Prelib transfers personal information to third parties for processing on behalf of Prelib, Prelib shall enter into an agreement containing appropriate privacy protection terms before any information is transferred to the third party. These terms must provide that the third party will not use the personal information for its own purposes, will keep the personal information secure, and will only process the information on the instructions of Prelib.

In addition, using certain third-party products to purchase products or services from Prelib may require us to provide your personal information to third parties in order to process the transaction.

7. RECORD RETENTION OF PERSONAL INFORMATION

Prelib will only retain personal information for as long as it is necessary to process related transactions and in accordance with the laws and regulations, including any Professional regulations, in force in the province of Quebec.

All personal information collected by Prelib is protected by security measures appropriate to the sensitivity of the information. All personal information stored in electronic form, including all information collected through the website of Prelib, is kept in confidential and secure data banks. Prelib evaluates its equipment and security procedures regularly and modifies them if required, so it can remain at the leading edge of security technology.
